原发性高血压病是心血管病的主要危险因素之一,随着年龄增加,原发性高血压病的发病率增加。本文的研究发现,无临床心血管疾病的健康志愿者随年龄增加动脉弹性功能降低,原发性高血压病人与年龄匹配的健康志愿者相比较,其动脉弹性功能进一步降低,证实了高血压病患者存在着更严重的血管损害或内皮功能障碍,亦提示内皮功能障碍可能是原发性高血压病的原因而不是结果。近来的研究表明,双亲为原发性高血压病的后代尽管其血压在正常范围内,却被发现已存在内皮功能障碍也支持这一假说[12]。研究表明,高血压内皮功能异常导致NO产生减少或生物利用度降低[8~11],从而致动脉弹性受损。
总之,正常老化过程伴随着动脉弹性功能降低,在高血压状态下,动脉弹性功能进一步降低,这表明多重心血管危险因素协同加重了动脉的损害,动脉弹性功能降低可作为血管损害的危险标志,本研究结果表明,增龄和高血压病导致动脉弹性功能损伤,动脉弹性功能检测为临床评价血管结构与功能损伤及其危险分层提供了一种替代方法。
